What is Hospice?

It’s a philosophy of caring for terminally ill patients, providing guidance for Palliative Care and support to their families and loved ones. Choosing hospice means choosing to optimize quality of life, but it also takes an honest realization that we are no longer trying to cure the disease, but working to minimize the symptoms of the disease and keep you or your loved one comfortable.

What is Palliative Care?

Palliative care concentrates on managing pain, controlling symptoms, and providing psycho-emotional and spiritual support.

It is an affirmation of the meaning of life, not giving up on it. It allows you to optimize the time you have, control symptoms, and do what needs to be done.

Team Approach to Patient Care

At Creed Hospice Inc., our approach to care is centered around our clients and their unique needs. Our dedicated team of skilled professionals works collaboratively to ensure every aspect of their journey is met with compassion and expertise. From expert nursing care to personalized assistance, we tailor our services to provide comfort and peace of mind.

Attending Physician
  • A licensed doctor designated by the patient as responsible for their medical care.
  • Participating member of the interdisciplinary team.
Medical Director
  • Co-certifies terminal prognosis with the attending physician.
  • Participating member of the interdisciplinary team.
  • Participates in the development of the individual plan of care.
  • Consults with other physicians regarding pain and symptom management as requested.
Skilled Nursing
  • Identifies patient’s/family’s physical, psychosocial, and environmental needs.
  • Addresses symptom management and comfort care needs.
  • Supports and instructs the family.
  • Documents problems, appropriate goals, interventions, and patient/family response to the plan of care.
  • Coordinates patient/family services.
Certified Home Health Aide
  • Assists the patient with hygiene, feeding, light housekeeping, and similar personal care activities as determined by the individual plan of care.
Supportive Therapies
  • Provide nutritional counseling, wound care, and other therapies as directed by the interdisciplinary team.
Social Worker
  • Provides counseling and linkage to community services for patients/families to assist them in developing optimal coping strategies.
Spiritual Support Staff
  • Provides spiritual care services that are consistent with patient/family beliefs and desires, communication, and support.
  • Consultation and education for patient/family and for interdisciplinary team members including grief and bereavement services.
Volunteers
  • Assist the health care team in providing optimum quality of life to terminally ill patients and their families.
